Tibial periostitis is inflammation of the periosteum, the membrane that covers the tibia. It is caused by excessive stress on the tibia and primarily affects runners. This intense pain may require a temporary break from athletic activity.

Symptoms of tibial periostitis

Symptoms and warning signs of tibial periostitis include:

Intense pain at the front, inside, outside or even back of the lower leg
Shin pain while walking, running or performing plantar-flexion movements that subsides at rest
Shin pain at night
Pain on palpation
Swelling in the painful area

Tibial periostitis can be diagnosed through a clinical examination or X-ray. It differs from a fracture and may reveal nodules during palpation, limiting use of the forefoot and toes.

Causes of tibial periostitis and people at risk

Tibial periostitis often occurs after intense physical activity, especially without adequate preparation. Athletes, including runners, sprinters, soccer players and skiers, are most often affected, but other factors include:

Direct trauma to the bone or a previous injury, such as a sprain
Unsuitable footwear
Running downhill or on hard surfaces
Body structure: excess weight or a difference in limb length

How to treat tibial periostitis
